For this agreement, waiver, and release, “PPETN” shall mean the Professional Photographers of East Tennessee and its directors, board, officers, volunteers, representatives, assigns, and agents.
“The Event” shall mean the event I am signing up for or any other PPETN meeting I attend.
I ASSUME THE RISK OF PARTICIPATING AND/OR VOLUNTEERING IN THE EVENT, including, but not limited to, any risks that may arise from negligence or carelessness on the part of PPETN, from dangerous or defective equipment or property owned, maintained, or controlled by PPETN, or because of their possible liability without fault. If, during the event, I observe unusual or unnecessary hazards, I will bring the attention of the event or hazard to the nearest event worker as soon as practicable and take appropriate action on my part to secure my safety.
I INDEMNIFY AND HOLD HARMLESS PPETN from all liabilities or claims made due to participation in this Event, whether caused by the negligence of PPETN. I acknowledge that PPETN is NOT responsible for the errors, omissions, acts, or failure to act of any party or entity conducting a specific event or activity on behalf of PPETN.
I WAIVE, RELEASE, AND DISCHARGE FROM ANY AND ALL LIABILITY, including but not limited to liability arising from the negligence or fault of PPETN, for my death, disability, personal injury, property damage, property theft, or actions of any kind which may hereafter occur to me including my traveling to and from the Event.
I CERTIFY that a qualified medical professional has not advised me not to participate. I certify that no health-related reasons or problems preclude my participation in the Event. I acknowledge that this Accident Waiver and Release of Liability Form will be used PPETN and that it will govern my actions and responsibilities at said activity or event.
I UNDERSTAND AND ACKNOWLEDGE THAT THIS ACTIVITY OR EVENT MAY INVOLVE PHYSICAL ACTIVITY and may carry the potential for death, serious injury, and property loss. I hereby consent, if unconscious, to receive medical treatment that may be deemed advisable in the event of injury, accident, and illness during the Event. The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law.
I UNDERSTAND AND ACKNOWLEDGE THAT I MAY BE PHOTOGRAPHED AT THIS EVENT or related activities. I agree to allow my photo, video, or film likeness to be used for any purpose by PPETN.
I UNDERSTAND AND ACKNOWLEDGE THAT ANY REGISTRATIONS ARE TRANSFERABLE BUT NOT REFUNDABLE. Circumstances at the scheduled time might require the Event to be canceled, and if so, registration fees would be refunded.
I CERTIFY THAT I AM 18 YEARS OF AGE OR OLDER.
I UNDERSTAND AND ACKNOWLEDGE that this waiver and release applies to any minor children under my care and supervision during the Event, and that I am solely responsible for the safety of said minor children. I certify that I hold any and all permissions from the minor children’s parents or legal guardians for their participation in the Event and that I am authorized to make decisions on their behalf, including but not limited to the waiver and release contained herein as well as medical decisions in the event of injury. I further UNDERSTAND AND ACKNOWLEDGE that I will always remain in the presence of said minor children during the Event.
I understand that I may be required to sign an additional, more specific waiver at the event.
I CERTIFY THAT I HAVE READ THIS DOCUMENT AND THAT I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT, AND I AGREE TO IT OF MY OWN FREE WILL.
